Scheduling

Appointments are required. Walk-ins are not accepted.

Appointments may be scheduled by phone, text, or email.

Appointments are confirmed once you receive confirmation from Precision Form Assistance.

Arrival

Please arrive on time.

If you arrive more than 15 minutes late, your appointment may need to be rescheduled.

Bring all required documents, identification, and any paperwork related to your matter.

What to Bring

• All forms you have received
  • Any letters from the court, DFS, or housing office
  • Identification (if required for your forms)
  • Supporting documents (pay stubs, leases, court notices, etc.)
  • Payment for services

Failure to bring required documents may result in rescheduling.

Payment Policy

Payment is due at the time services are provided.

Accepted payment methods: cash or card

Services are billed for clerical and administrative assistance only. Payment does not include legal advice or representation.

Cancellation Policy

Appointments must be canceled at least 24 hours in advance.

Failure to cancel may result in refusal of future appointments.

If you need to reschedule, please contact us as soon as possible.

Abusive, threatening, or inappropriate behavior will result in immediate termination of the appointment.
